A birth plan is a document that states your delivery wishes. A birth plan is a communication tool. The plan should be simple, clear and concise. The plan is kept by the midwife or in the medical status of the hospital. Click here to download an example.
If you have made a birth plan is it recommended to take this with you when you come to our practice for consultation and that you e-mail this document to firstname.lastname@example.org
We will go through the plan together and discuss your wishes so that everything is clear for us. Make sure your plan is also in your booklet together with your pregnancy card. This way it is easier for the midwife who comes to your house for your delivery to read the plan. It is also easier to take with you to the hospital if necessary and give to the hospital staff who will attend to you from then on.
